Please read them carefully before using our site.

If you do not agree with any part of these Terms and Conditions, please do not use our website or submit any forms through it. If you are accessing this website on behalf of a minor child, you represent that you are the parent or legal guardian of that child and that you accept these terms on their behalf.

We reserve the right to update these Terms and Conditions at any time. When we do, we will update the effective date at the top of this page. Your continued use of the website after any update constitutes your acceptance of the revised terms. We encourage you to review this page periodically.

About Sprouting Oaks Foundation

Sprouting Oaks Foundation is a Texas nonprofit corporation in formation, working toward 501(c)(3) tax-exempt status. Our mission is to provide free dental care, oral health education, and a network of professionals and donors committed to serving uninsured and underserved children in Williamson County, Texas, and beyond. We coordinate access to dental care services through partnerships with licensed dental professionals and community organizations. We are not a dental practice, a licensed healthcare provider, or a covered entity under the Health Insurance Portability and Accountability Act (HIPAA) at this stage of our operations.

Donations

Donations made through this website are processed by a secure third-party payment processor. By submitting a donation, you authorize the processing of your payment in the amount you specify. All donations to Sprouting Oaks Foundation are made voluntarily and are non-refundable except in cases of documented processing error. If you believe a donation was processed in error, please contact us promptly at contact@sproutingoaksfoundation.org, and we will review the matter in good faith.

Sprouting Oaks Foundation is a nonprofit corporation working toward 501(c)(3) tax-exempt status with the Internal Revenue Service. Until that status is confirmed, donations may not be tax-deductible. Once our 501(c)(3) status is confirmed, qualifying donations will be tax-deductible to the extent permitted by law, and donors will receive written acknowledgment of their gifts.

 

We will notify donors and update our website when tax-exempt status is obtained. We encourage you to consult a tax advisor regarding the deductibility of your contribution.

 

We are committed to using donations responsibly and in furtherance of our charitable mission. While we honor donor intent as much as operationally possible, Sprouting Oaks Foundation reserves the right to direct unrestricted donations to the areas of greatest organizational need. If you wish to designate a donation for a specific purpose, please contact us before donating.

Program and Event Registration

By submitting a registration or intake form through this website, you confirm that the information you provide is accurate and complete to the best of your knowledge. You understand that program eligibility is subject to verification, including confirmation of age, residency, and insurance status, and that submission of a registration form does not guarantee placement in a specific event. We will contact you to confirm your registration and provide event details.

 

If you are registering a minor child for our dental care services, you represent that you are the parent or legal guardian of that child and that you have the authority to consent to their participation and to the collection and use of their information as described in our Privacy Statement. Participation in Sprouting Oaks Foundation programs requires a completed consent and authorization form to be signed by a parent or legal guardian prior to any services being rendered. Consent forms will be provided separately at or before each event.

 

You may cancel or withdraw a registration at any time by contacting us at contact@sproutingoaksfoundation.org. We ask that you notify us as early as possible so we can offer the appointment to another child in need.

Health and Dental Services Disclaimer

Sprouting Oaks Foundation coordinates access to free dental care services for uninsured and underserved children through partnerships with licensed, independent dental and dental hygiene professionals. The dental services provided at our events are rendered by those independent licensed professionals, not by Sprouting Oaks Foundation as an organization.

Nothing on this website constitutes dental advice, medical advice, diagnosis, treatment, or a professional clinical recommendation. The information on our website is provided for general informational purposes about our programs and mission only. It is not a substitute for professional dental or medical consultation, diagnosis, or treatment. You should always seek the advice of a licensed dental or healthcare provider with any questions you have regarding your child's oral health or any dental condition.

Sprouting Oaks Foundation does not guarantee specific clinical outcomes from services provided at our events. The dental professionals who volunteer at our events exercise their own independent clinical judgment, and their participation does not create an employment or agency relationship with Sprouting Oaks Foundation. Any clinical decisions made during our events are the sole professional responsibility of the licensed provider rendering care.

In the event of a dental or medical emergency, please call 911 or proceed to the nearest emergency room. Our events are not equipped to handle medical emergencies, and this website is not an emergency resource.

Volunteer Participation

By submitting a volunteer inquiry or application through this website, you express your interest in volunteering with Sprouting Oaks Foundation. Submission of an inquiry does not constitute acceptance as a volunteer. All volunteer participation is subject to a separate volunteer agreement and, where applicable, background screening and verification of professional credentials.

Volunteers participate in Sprouting Oaks Foundation events as independent volunteers, not as employees or agents of the Foundation. Sprouting Oaks Foundation is not responsible for injury, loss, or damage arising from volunteer participation except to the extent required by applicable law.

Partner and Provider Inquiries

By submitting a partner or provider inquiry through this website, you acknowledge that your submission initiates a preliminary conversation only. It does not create a binding agreement, partnership, employment relationship, or obligation of any kind on the part of either party. Any formal partnership or provider engagement will be governed by a separate written agreement executed by authorized representatives of both parties.
